Practice Policies

I authorize the clinic and its associated health professionals to collect my personal and medical information [as documented above]. In addition, I authorize the clinic and its associated health professionals to communicate with my family doctor and/or referring doctor as deemed necessary for my beneficial treatment. I also understand that my personal and medical information is confidential and will only be disclosed to third parties with my [would this be “your” permission?].

Lewis Physical Therapy requires that you put a card on file for contactless payments. This helps keep both you and us safe, as well as spend more time with you, rather than taking a payment after your session.

Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the therapists’ day that could have been filled by another patient. As such, we require 24 hours’ notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours’ notice, or miss their appointment, will be charged a cancellation fee to the card on file.


Lewis Physical Therapy is an out-of-network provider. Reimbursement for your costs would fall under your out-of-network benefits. We will provide you with a “superbill” for reimbursement purposes. Coverage varies quite a bit, depending on your plan. Generally, most PPO plans will cover out-of-network care.

At Lewis Physical Therapy, we prioritize building a strong partnership with our patients. Rather than relying on insurance companies, we choose to have a direct contract with you. This allows us to offer focused one-on-one care during fewer, longer appointments, ensuring we fully understand and address your specific needs.

Unlike typical in-network providers, we dedicate 53 minutes of uninterrupted one-on-one care with every patient. This extended session time allows for a thorough assessment and effective treatment planning, which can lead to faster healing and reduced healthcare costs in the long run.

As an out-of-network provider, we are not bound by treatment visit limitations, therapy caps, or prior authorizations, allowing us to focus solely on your well-being.

A $25 service charge will be charged for any checks returned for any reason.

Active patients will be given my personal phone number. If you need to contact me between sessions, please leave a message on my voicemail. I am often not immediately available; however, I will attempt to return your call within 24 hours. If this is a true emergency, please call 911 or any local emergency room instead of calling Lewis Physical Therapy or your doctor.

Due to the importance of your confidentiality and the importance of minimizing dual relationships, I do not accept friend or contact requests from current or former clients on any social networking site (Facebook, LinkedIn, etc.). I believe that adding clients as friends or contacts on these sites can compromise your confidentiality and our respective privacy. It may also blur the boundaries of our therapeutic relationship. If you have questions about this, please bring them up when we meet and we can talk more about it. However, I do have a professional Facebook page which you are welcome to view and follow.


I cannot ensure the confidentiality of any form of communication through electronic media,
including text messages.

Please communicate via email for any scheduling, health or billing related issues. You are welcome to use text messaging to announce your arrival to appointments or to cancel a visit, etc. (if you must cancel on short notice, it is best to call me directly and leave a message if I can’t take the call).

While I may try to return messages in a timely manner, I cannot guarantee immediate response and request that you do not use these methods of communication to discuss therapeutic content and/or request assistance for emergencies.

If you are a minor, your parents may be legally entitled to some information about your
treatment. I will discuss with you and your parents’ what information is appropriate for them to receive and which issues are more appropriately kept confidential.


After your initial evaluation, we will discuss the recommended duration of physical therapy for your condition.

Pre or post-partum wellness visits can take a minimum of 3 visits, but more if you have one of the following issues:

Generally, pain conditions can require up to 12 visits, sometimes more.
Incontinence issues can take 6-10 visits (less for stress, more for urgency).
Constipation/retention can require up to 10 visits.
Prolapse issues can take 4-6 visits unless it also includes incontinence or constipation (see above).
Diastasis Recti or Post -partum back pain could require 4-6 visits.
These numbers may be modified to be shortened or lengthened as necessary (less, if you recover more quickly than usual, for instance).

As we commence treatment, you will be recommended to schedule your entire visit series. This will ensure that you have enough treatments “on the books” to bring you to a level of higher function and comfort. After completing your series of scheduled visits, you may need more than the scheduled series. If we do schedule more, you will then be on a wait list to get scheduled in at times available, such as by cancellations. I can assure you that you will be able to manage more independently at that point and will continue to progress even if you do not have a session every week.
